Are you a ministry leader at FBC who must turn in an Activity Forms from time to time? If you answered yes, then we have an exciting new feature available for your convenience.
You can now access and submit the Activity Request Form and the Activity Summary Form online!
Simply go to the Risk Management page of our website and click the link for Activity Request Form or for the Activity Summary Form. The pdf version of each form is still available if you prefer to print it and fill it out the 'old-fashioned' way.
